How NHP works
We’re with you every step of the way on your journey to recovery. You can contact NHP for general information and referral to resources without enrolling in the program.
Entering the Program

Enrolment and assessment
After your initial contact with NHP, you may be contacted by a case manager for an initial assessment.
The assessment determines your eligibility and the nature of the treatment(s) you may need. It is also used to create a plan for treatment and monitoring.
At no time will your health disorder be publicly disclosed. As long as you follow your obligations to the program, information about your participation is not made public.
Treatment
NHP will develop a treatment plan to meet your needs. Throughout the program, your case manager will support you in your recovery and, if needed, help you to find an approved treatment provider.
Monitoring
Your case manager will work with you to develop a monitoring plan based on your treatment needs.
Exiting the program
Once you successfully reach the end of your treatment and monitoring plan you may leave the program.
Your responsibilities
When you decide to enter NHP, your case manager will review your responsibilities for following a treatment and monitoring plan.
Treatment fees
Program related costs will vary depending on your needs. Your case manager will explain specific costs and how they are covered. You will not be charged for the NHP case management services and any independent medical examinations requested by the program.
Length of program
The length of the program varies depending on your needs. The program length is based on best practices and research that demonstrates what is needed to support treatment and recovery from substance use and/or mental health disorders.
Monitoring and adherence
As a participant in NHP, you are assigned a case manager who will be available to answer any questions you have about your monitoring plan. Questions about adherence to your plan, including the potential for relapse, can be discussed with your case manager.
Privacy and confidentiality
Your privacy is important. No one has access to any of your information without a signed consent form outlining specifically what you are consenting to. NHP adheres to the professional standards of confidentiality as outlined in the Personal Health Information Protection Act, 2004 (PHIPA). Our case managers are available to answer any questions or concerns you have regarding the collection, use and dissemination of any personal and/or health information you disclose.
